Changes to Proof of Vaccination Requirement

Effective December 20, 2021, proof of vaccination and valid government ID will now also be required for youth aged 12 to 17 years to enter recreation facilities for their sport, per Ontario Regulation 364/20. Everyone (12 and older, except those with a valid medical exemption) entering our recreation facilities will be required to:

• show proof of vaccination (QR code version required as of January 4, 2022)

• show government identification (a picture or legible photocopy is acceptable)

• complete the COVID-19 health screening

• complete the contact tracing form

These requirements are already in place for volunteers, parents and spectators (12 and older).

If your 12-17 year old child is registered in a city program and is not fully vaccinated, they will not be able to participate as of December 20, 2021. Please email prc@cambridge.ca to withdraw from the program.

Proof of vaccination and identification will be required each and every time to enter the facility.

Use of enhanced vaccine certificate with QR code

Starting January 4, 2022, only the enhanced vaccine certificate with QR code can be used as proof of vaccination to enter any recreational facility. The QR code proof can be used digitally (on your phone) or by printing a paper copy. Individuals can download their enhanced certificate with QR code by visiting covid-19.ontario.ca/get-proof. Instructions for how to save it to your phone are on the Province's website.

Starting January 10, 2022, city recreation facilities will no longer accept physician notes for proof of medical exemption. Residents with a medical exemption will need to present a provincially-issued exemption QR code for verification. Further information about the exemption QR code process can be found on the province’s website.

facilities. Regulations are subject to change based on further direction and/or guidance from the Province and Regional governments. Program participants are reminded to Check Before you Rec for health screening and applicable program information.

We will continue to monitor provincial regulations and announcements, and are working closely with Region of Waterloo Public Health Unit to interpret regulations and guidelines as they are released or amended.
